I am willing to make Driver Education my #1 priority over ALL extracurricular activities.*

I have read all the Terms & Conditions for Driver Education.*

I understand full payment is due at time of registration to complete the process.*

I understand there is a Mandatory Parent/Guardian Meeting via Zoom. Failure to attend results in an automatic drop from the course. My parent/guardian is aware of the date and time.*

I understand there are 2 parts to Driver Education: 30 hours of Classroom time and 6 Behind the Wheel drive times (2 hours each - 6 hours of driving and 6 hours of observation, 12 hours total)*

I have checked all the classroom days & times and there is zero conflict. Behind the Wheel time is in addition to classroom time and won't be scheduled until after the Mandatory Parent Meeting. The schedule is based on Instructor availability and is first come first serve. I understand I might not get the day and time I prefer.*

I understand Driver Education courses are in high demand and registration fills up quickly and I might not get into the course. *

My parent(s)/guardian(s) and I are ready to partner with Southern Oregon Driver Education, Inc. to attend all the classes AND behind the wheel drives with a positive attitude and ready to learn. Together we can help make our roads and community safer.*

I have a valid permit. I will enter my permit #, issued date, and expiration date on the registration.*
